Here you can find registration forms that are needed to secure your child's space in our wonderful programs! The It's a Small World Enrollment/Registration form pertains to the Small Wonders, Tireless Toddlers, and Little Adventurer's Preschool, and Pre-Kindergarten programs, while the Explorer Club registration form pertains to the School Aged Programs. The registration form and registration fee are submitted to the office together to hold a space.
It's A Small World
The below forms are required to be submitted to the office no later than 24 hours prior to your child's first day.

Registration Form (Submitted to Director with Registration Fee after Scheduled Tour)

Health Appraisal Form (Most updated health appraisal form, printed and signed by a physician. If your child's primary care provider uses a different formatted form, please submit their form as long as it is singed, dated, and includes immunization record and physical)

Recommended Immunization schedule (If you have chosen an alternative schedule or to waive out of any immunizations, an annual waiver form will need to be submitted to the office. Visit: Washtenaw County Public Health for more information or to schedule a waiver appointment)

Medication Form (This form is needed to administer any medications to your child while they are in our care. This includes but is not limited to, sun block, bug spray, breathing treatments, aspirin, cough syrup, ect. please print and fill out this from, and bring it in with the specified medication.)

Summer Camp Field Trips: All children enrolled for summer camp go to the pool every Tuesday and Friday. All children will go on the "No Wifi" fieldtrip to the park every Tuesday. Cubs & Grizzlies (ages 5-8) will go on field trips on Thursdays. Jr’s. and Sr’s (ages 9 and up) will go on field trips on Wednesdays.

C.I.T. Registration: Our Counselor In Training Program is for our 7th- 12th grade summer camp children. Application and registration must be submitted and accepted for CIT rate.
